Name, Address, And Other Records Changes Failure
to notify the Kelley Direct Online MBA Program Office
on a timely basis of record changes often results
in missed communication, including canceled registrations,
delayed certification for graduation, and in some
cases, being placed on inactive status. All candidates
should report any changes in name, address, telephone
number, place of employment, or marital status to the
Kelley Direct OnLine MBA Program Office immediately.
The Online MBA Office can report any address or telephone
number changes for you by filling out a “Student
Record Change” form with the registrar but you are required
to report any name changes directly to the registrar.
You may report any changes with the Registrar/Bursar through
the INSITE
website. If you choose to make these changes through INSITE,
please be sure to also contact the Kelley Direct
Online MBA Program Office with these changes.
